What is Semax?
Semax is a seven-amino-acid synthetic peptide built from the 4 to 10 fragment of adrenocorticotropic hormone (ACTH), with a Pro-Gly-Pro tail added. The tail makes it far more resistant to enzymes and removes the hormonal action of the parent molecule, so the fragment keeps its activity in the brain without the effects of ACTH on the adrenal glands. It was developed in Russia, where it has been in clinical use since the 1990s.
What has Semax research shown?
The most consistent thread in Semax research is brain-derived neurotrophic factor (BDNF), a protein central to how the brain forms and reshapes connections. Two human studies stand out:
- Stroke recovery, 2018. In 110 patients after ischaemic stroke, Semax raised plasma BDNF, which stayed high for the whole study whatever the timing of rehabilitation. Semax and higher BDNF levels sped functional improvement and improved final Barthel Index scores, a standard measure of independence in daily activities, and early rehabilitation went with better motor performance.
- Brain networks, 2018. In a resting-state functional MRI study of 24 adult volunteers (11 men and 13 women), intranasal Semax was compared with placebo, with scans before and 5 and 20 minutes after. The Semax group showed a larger volume of the frontal (medial frontal cortex) part of the default mode network than the placebo group.
