Tirzepatide vs Semaglutide: What the Head-to-Head Trials Show
Weight Loss & Metabolism Research
Where tirzepatide and semaglutide have been tested against each other, tirzepatide has come out ahead on both blood sugar and body weight. Semaglutide (sold as Ozempic) acts on one gut-hormone receptor; tirzepatide acts on two. This is the tirzepatide vs semaglutide record: the two head-to-head trials, the analyses built on them, and what the mechanism studies suggest about why.

- In a 40-week New England Journal of Medicine trial of 1,879 adults with type 2 diabetes (2021), every tirzepatide dose lowered HbA1c more than semaglutide: by 2.01 to 2.30 percentage points, against 1.86, with 1.9 to 5.5 kg more weight loss.
- In a 72-week trial of 751 adults with obesity in the same journal (2025), weight fell 20.2% with tirzepatide against 13.7% with semaglutide.
- Among the 425 participants in that trial who started with prediabetes, 89.9% on tirzepatide returned to normal blood sugar, against 76.2% on semaglutide.
- In a 28-week double-blind mechanism trial, tirzepatide improved both insulin secretion and insulin sensitivity more than semaglutide.
Tirzepatide vs semaglutide: two receptors against one
Both peptides are built on incretins, the gut hormones released after a meal that prompt the pancreas to release insulin. Semaglutide is a selective GLP-1 (glucagon-like peptide-1) receptor agonist. Tirzepatide is a dual agonist: it activates the GLP-1 receptor and the receptor for GIP (glucose-dependent insulinotropic polypeptide), the second incretin. Both were given once a week in the head-to-head trials.
| Tirzepatide | Semaglutide | |
|---|---|---|
| Receptors | GIP and GLP-1 | GLP-1 only |
| HbA1c drop, diabetes trial | 2.01 to 2.30 points | 1.86 points |
| Weight loss, obesity trial | 20.2% | 13.7% |
Reta (GLP-3) adds a third receptor, glucagon. It has its own comparisons: reta (GLP-3) vs semaglutide and reta vs tirzepatide.
Blood sugar: the type 2 diabetes head-to-head
The direct test in type 2 diabetes is a 2021 New England Journal of Medicine trial. It randomised 1,879 adults (average age 56.6, average HbA1c 8.28%, average weight 93.7 kg) to one of three tirzepatide doses or to semaglutide, the peptide sold as Ozempic for type 2 diabetes, for 40 weeks in an open-label design. The primary endpoint was the change in HbA1c, a marker of longer-term average blood sugar.
Every tirzepatide dose was superior to semaglutide on that endpoint, the lowest dose included, and every dose produced more weight loss. Semaglutide lowered HbA1c by 1.86 points; tirzepatide went further:
| Tirzepatide dose | Extra HbA1c drop | Extra weight loss |
|---|---|---|
| Lowest | 0.15 points | 1.9 kg |
| Middle | 0.39 points | 3.6 kg |
| Highest | 0.45 points | 5.5 kg |
More participants reached their targets
A University of Porto team re-analysed the shared trial data in 2026 (Diabetologia) against four standard care targets: HbA1c under 7%, blood pressure under 140/90, LDL cholesterol under 1.8 mmol/L and more than 10% weight loss. Three or more were met by 42%, 53% and 57% on the three tirzepatide doses, against 34% on semaglutide. On a stricter set of targets, the figures were 15%, 20% and 29%, against 8%. Tirzepatide raised the odds of reaching an HbA1c under 6.5% (odds ratio 1.88) and of losing more than 15% of body weight (odds ratio 3.86).
Consistent whatever the starting point
A 2024 analysis of the same trial (J Clin Endocrinol Metab) grouped participants by how well their insulin-producing beta cells worked and how insulin-resistant they were at the start. A marker of beta-cell function rose 96.9 to 120.4% on tirzepatide, against 84.0% on semaglutide, and a marker of insulin resistance fell 15.5 to 24.0%, against 5.1%. HbA1c and weight fell further with every tirzepatide dose in every group.
Weight: the obesity head-to-head
The weight comparison came in 2025, again in the New England Journal of Medicine: 751 adults with obesity but without type 2 diabetes, randomised to tirzepatide or semaglutide for 72 weeks in an open-label design. Each participant took the highest dose of their assigned peptide that they tolerated.
Weight fell 20.2% with tirzepatide against 13.7% with semaglutide, and waist circumference fell 18.4 cm against 13.0 cm. More participants on tirzepatide reached every weight-loss mark the trial tracked: at least 10%, 15%, 20% and 25%.
The gap opened early: by week 24, 44% of the tirzepatide group had already lost at least 15% of their body weight, against 21% on semaglutide (American Journal of Medicine, 2026).
Blood sugar without diabetes: the prediabetes subgroup
425 participants in the obesity trial started with prediabetes (average HbA1c 5.86%, average BMI 40), and a 2026 analysis (J Endocrinol Invest) tracked their blood sugar. HbA1c fell 0.60 points on tirzepatide against 0.48 on semaglutide, and 89.9% of the tirzepatide group reverted to normal blood sugar (an HbA1c under 5.7% with normal fasting glucose), against 76.2% of the semaglutide group. Weight in this subgroup fell 21.5% against 14.5%, with greater improvements in fasting insulin and insulin resistance on tirzepatide.
Why tirzepatide pulls ahead: the mechanism studies
To find out what drives the difference, a 28-week double-blind trial at two German centres (Lancet Diabetes and Endocrinology, 2022) randomised 117 adults with type 2 diabetes on metformin to tirzepatide, semaglutide or placebo, and measured insulin action with clamp tests. Tirzepatide improved the clamp disposition index, which combines insulin secretion and insulin sensitivity, significantly more than semaglutide, with gains in both components. After a test meal, blood glucose rose less on tirzepatide, with lower insulin and glucagon, and these effects were greater than with semaglutide.
Follow-up analyses of that trial added detail. Tirzepatide lowered fasting glucose, and glucagon across a meal test, more than semaglutide (J Clin Endocrinol Metab, 2024). Insulin sensitivity improved more per unit of weight lost on tirzepatide, and the authors concluded the larger gain was not simply down to greater weight or fat loss (Diabetes, Obesity and Metabolism, 2025). Tirzepatide also produced greater fat-mass reduction, while appetite fell to a similar degree on both peptides, so the authors point to energy use across the day as the next question (Diabetes Care, 2023).
Outside the trials: real-world and pooled data
The pattern holds in routine care. A 2024 JAMA Internal Medicine cohort drew on US health-system records, matching 18,386 adults with overweight or obesity who started the diabetes-labelled form of either peptide. People on tirzepatide were significantly more likely to reach 5%, 10% and 15% weight loss (hazard ratios 1.76, 2.54 and 3.24). Weight loss on treatment was greater with tirzepatide by 2.4 percentage points at 3 months, 4.3 at 6 months and 6.9 at 12 months, and gastrointestinal side effects occurred at similar rates.
A 2026 meta-analysis of 12 head-to-head studies, pooling trials and real-world cohorts (Obesity Research and Clinical Practice), found 4.61 percentage points more body-weight reduction with tirzepatide, or 4.76 kg, and higher odds of reaching every threshold from 5% to 20%.
Side effects in the head-to-heads
In both trials the most common side effects were gastrointestinal and mostly mild to moderate; in the obesity trial they occurred mainly while doses were being stepped up. In the diabetes trial the rates were close: nausea in 17 to 22% on tirzepatide against 18% on semaglutide, diarrhoea in 13 to 16% against 12%, and vomiting in 6 to 10% against 8%. Low blood sugar below 3.0 mmol/L was uncommon on both (0.2 to 1.7% against 0.4%), and serious adverse events were reported in 5 to 7% against 3%.

Frequently asked questions
What has research on tirzepatide versus semaglutide shown?
In both head-to-head trials, tirzepatide outperformed semaglutide: on HbA1c and weight in 1,879 adults with type 2 diabetes (NEJM, 2021), and on weight and waist size in 751 adults with obesity (NEJM, 2025).
What did the head-to-head trials show for blood sugar?
Every tirzepatide dose was superior to semaglutide on HbA1c in type 2 diabetes, and more participants reached HbA1c targets (odds ratio 1.88 for an HbA1c under 6.5%). In adults with obesity and prediabetes, 89.9% on tirzepatide returned to normal blood sugar, against 76.2% on semaglutide.
What did the head-to-head trials show for weight?
In obesity without diabetes, weight fell 20.2% on tirzepatide and 13.7% on semaglutide over 72 weeks, and waist circumference fell 18.4 cm against 13.0 cm. In type 2 diabetes, tirzepatide produced 1.9 to 5.5 kg more weight loss than semaglutide over 40 weeks.
What has research shown about why tirzepatide outperformed semaglutide?
Tirzepatide activates two incretin receptors, GIP and GLP-1, where semaglutide activates GLP-1 alone. In a 28-week double-blind trial of 117 adults with type 2 diabetes, tirzepatide improved both insulin secretion and insulin sensitivity more than semaglutide, and blood glucose rose less after a meal.
What side effects did the head-to-head trials report?
Mainly gastrointestinal effects such as nausea, diarrhoea and vomiting, mostly mild to moderate and at similar rates on both peptides. In the obesity trial they occurred mainly while doses were being stepped up.
