Tirzepatide is a once-weekly prescription peptide that switches on two gut-hormone receptors, GLP-1 and GIP. Older compounds such as semaglutide (Ozempic) switch on GLP-1 alone. That second pathway is a big part of why tirzepatide has become one of the most studied metabolic peptides of the decade, and why research now reaches well beyond blood sugar.
Tirzepatide and type 2 diabetes
The headline result is a 40-week head-to-head trial in 1,879 adults with type 2 diabetes, published in the New England Journal of Medicine in 2021. Tirzepatide came out ahead of semaglutide (Ozempic) at every dose tested. HbA1c is the blood test for your average blood sugar over the last three months.

Tirzepatide and inflammation
Type 2 diabetes comes with a constant, low-level inflammation in the body. You can’t feel it, but over the years it wears down blood vessels, kidneys and joints. A study in Diabetes, Obesity and Metabolism ran blood tests on adults with type 2 diabetes over 26 weeks of tirzepatide and found inflammation dropped on several fronts:
- The standard inflammation blood test (the one a doctor orders to check for inflammation) came down.
- Inflammation in the blood vessel walls came down, which matters because that is where heart disease starts.
- Signs of inflamed, stressed tissue came down.
- Stress signals from fat tissue came down too.
The best part: much of this happened in the first four weeks, before most of the weight had come off. That points to tirzepatide calming inflammation directly, not just as a side effect of weight loss.
Tirzepatide and fatty liver
Fatty liver is common with type 2 diabetes, and the serious version is when that fat makes the liver inflamed and starts to scar it (doctors call this MASH). A 52-week study of 190 adults with an inflamed, scarring fatty liver, published in the New England Journal of Medicine in 2024, found the liver inflammation cleared up in 44% to 62% of people on tirzepatide, compared with 10% on placebo. Tirzepatide and blood pressure
A 2024 study in Hypertension had 494 adults wear blood-pressure monitors around the clock. Over 36 weeks, the top number (systolic pressure) fell by 7 to 11 points compared with placebo. That’s about what a standard blood-pressure pill does.
