Findings
- 10 mg in 5 mL, about 28 sprays at 350 mcg per spray.
- Thymosin beta-4 binds actin, the protein cells use to move and rebuild.
- In a 2015 eye trial, thymosin beta-4 drops cut ocular discomfort by 35.1% and corneal staining by 59.1% against vehicle.
- Also sold with BPC-157 as the BPC-157 + TB-500 Nasal Spray.
What is TB-500?
Thymosin beta-4 is a naturally occurring protein present in nearly every cell and released by platelets and many other cells after injury. It binds actin, the scaffolding protein cells use to move and rebuild, which is why its research sits in cell migration, wound closure and new blood-vessel growth. TB-500 is the synthetic peptide based on its active region. Our article on TB-500 and thymosin beta-4 covers the background.
What has thymosin beta-4 research found?
- Severe dry eye, 2015. In a small randomised, double-masked, placebo-controlled phase 2 trial of 9 patients at two US sites, thymosin beta-4 eye drops were safe and well tolerated. At day 56, 28 days after the last drops, the eyes given thymosin beta-4 showed 35.1% less ocular discomfort (P = 0.0141) and 59.1% less corneal fluorescein staining (P = 0.0108) than vehicle, with better tear film break-up time and tear volume.
- Dry eye in a challenge chamber, 2015. In a phase 2 trial that randomised 72 people with moderate to severe dry eye, discomfort during a controlled adverse environment challenge on day 28 was 27% lower than placebo (P = 0.0244), central and superior corneal staining improved significantly, and no adverse events were observed; the two primary endpoints did not reach significance.
- First-in-human safety, 2021. A randomised, double-blind phase 1 study of recombinant human thymosin beta-4 gave single infusions to 54 volunteers across seven cohorts and a 10-day daily schedule to 30 more. Adverse events were mild to moderate, there were no serious adverse events, and there was no obvious accumulation.
These trials used full-length thymosin beta-4 as eye drops or by infusion; TB-500 as a nasal spray has not yet been the subject of a published human study.

Frequently asked questions
How much TB-500 is in each spray?
About 350 mcg. The bottle holds 10 mg of TB-500 in 5 mL of sterile saline, about 28 sprays.
Is TB-500 the same as thymosin beta-4?
TB-500 is the research-catalogue name for the synthetic peptide based on the active region of thymosin beta-4. Clinical papers on the full protein write it as thymosin beta-4 or Tβ4.
What has thymosin beta-4 research shown?
In a 2015 randomised phase 2 trial in severe dry eye, thymosin beta-4 eye drops reduced ocular discomfort by 35.1% and corneal staining by 59.1% against vehicle, with the gains still present 28 days after the last drops.
Is there a nasal spray with TB-500 and BPC-157 together?
Yes. The BPC-157 + TB-500 Nasal Spray holds 10 mg of each peptide in 10 mL of sterile saline, about 57 sprays.
Does the TB-500 nasal spray need mixing?
No. It arrives ready-mixed in sterile saline in a sealed metered atomiser, so no reconstitution and no bacteriostatic water are needed.
